Company Policies

Effective December 1, 2023

Omorphia Aesthetics reserves the right to change & implement policies and charge fees as listed below. Prices, services and policies are subject to change.

CLIENT RESPONSIBILITY

Client is responsible for the payment of all services rendered. Payment is expected at the time of service for all charges towards current visit, as well as previous balances (i.e: late fees, no-show fees). We accept cash, Visa, Master Card, American Express, Discover, Apple Pay, and Cherry Payment Plans. Gratuity for services are never expected, but always appreciated.

CARD ON FILE POLICY

All appointments require a debit or credit card on file. If a Last Minute Notice/No Notice occurs, this card will be charged. Omorphia Aesthetics LLC, its employees, and contractors, do not have access to this information once it has been input into our system. All card information if kept on a secure server through our Payment Operating System. You may see a small charge of $0.50 -$1.00 has a temporary hold to ensure the card is active and has available funds. The charge will be reimbursed automatically.

DEPOSIT POLICY

Omorphia Aesthetics reserves the right to implement appointment deposits. Some services may require a deposit at the time of booking your appointment. Appointment deposits are non-refundable and serve to reserve our medical staff and our equipment specifically for you and your treatment. Any changes or cancellations to these appointments without the required amount of notice will forfeit the deposit in its entirety and maybe subjected to additional fees. Appointments may be changed without penalty if the minimum amount of notice is provided, per the table below. If you wish to include the deposit into a Cherry payment plan, you must be approved for the total amount needed and finalize your payment plan before you may book your appointment.

LAST MINUTE NOTICE/NO NOTICE FEE

If you cancel or reschedule within 24 hours of your appointment, fail to attend, or are more than 30 minutes late, you will be automatically charged a reservation fee of $50 on the card you keep on file. This policy is in place out of respect for our providers AND our clients. Cancellations with less than 24 hours notice are difficult to fill. By giving last minute notice or no notice at all, you prevent someone else from being able to schedule into that time slot, and leave a vacancy in your provider's schedule.

LATE POLICY

If you arrive to your appointment late, you may have to reschedule due to time constraints. Arriving to your appointment late, may reduce your total appointment time. If you are more than 30 minuets late to your appointment you will be charged a $50 Last Minute Notice/No Notice Fee.

*If you break this policy on two or more occasions, you will be able to request an appointment, rather than book, and may be required to pay for your next service in full at the time of booking. 

**To cancel, you may call the spa at 856-514-1983 during open hours or you may cancel through your client login online. Omorphia Aesthetics LLC is not responsible for missed calls or voicemails during closed hours.

REFUND POLICY

Your complete satisfaction is our first priority. In the unlikely event you are at all unsatisfied with your purchase, please let us know immediately. We accept returns for unopened and unused merchandise 7 days after purchase. Treatments and procedures are non-refundable. If you do not wish to complete your pre-paid service, we can give you a credit with Omorphia Aesthetics at the value paid minus services given at full price that can be used towards another service or product.

MULTIPLE SESSION SERVICES

We may recommend multiple sessions for certain services to ensure optimal results. Sessions can be paid for individually or you may purchase our predetermined bundles of multiple sessions. Bundles must be paid for in full, at the time of the first session. 

SUPERVISING PHYSICIAN

Omorphia Aesthetics, of Omorphia Aesthetics LLC, is a registered & licensed legal entity in the state of New Jersey. Omorphia Aesthetics follows all legal guidelines set forth by local, state and federal law. Omorphia Aesthetics, and all contracted providers/LLCs are in compliance with their applicable boards including but not limited to; the New Jersey State Medical Board, New Jersey Board of Nursing, and Board of Cosmetology. 

Omorphia Aesthetics LLC has a legally binding agreement with Cheryl Starrett Keller, MD enlisting her as a collaborating physician. All contractors and service providers have legally binding agreements on file with Omorphia Aesthetics LLC. Any and all contractors providing any service deemed medical by the state of New Jersey, have a legally binding agreement with Cheryl Starrett Keller, MD enlisting her as collaborative physician. The collaborative physician has authorized all providers to preform medical services where applicable by law based on their certifications, training and licensing. 

Omorphia Aesthetics LLC policies, procedures, and protocols are developed, authorized and established to follow all guidelines, requirements & laws set forth by the owner, collaborating physician, product/device manufacturers, and state requirements. All providers are knowledgable and practicing all safety and emergency protocols in compliance with the local, state and federal law.

RIGHT TO REFUSE

Omorphia Aesthetics reserves the right to refuse service. Omorphia Aesthetics will not discriminate against Race or color, National origin or citizenship status, Religion or creed, Sex, Age, Disability, pregnancy, or genetic information, veteran status. Certain services can not be preformed on everyone due to health risks and contraindications. 

HIPPA COMPLIANCE

Omorphia Aesthetics and all employees, contracted individuals, and service providers will abide by all HIPAA policies. 

MEDIA POLICY

Photographic documentation will be taken. As part of your treatment, we will be photographing the treatment area of your body/face (and in some cases, filming the treatment process). This will allow us to visually monitor your individual progress and see the results of your treatment over time. We reserve the right to publish any content that does not reveal your identity. In all cases, we will do everything we can to keep your identity anonymous. If any of the content is in question or does reveal your identity, we will always ask for your permission first.

We would appreciate your willingness to share your outcomes and results with others, for both training and marketing purposes within the beauty, cosmetic, and aesthetic industries.
